Streamlining clinician access to proven chronic care support programs
Challenge
Despite a growing shift toward digital access, parts of the healthcare sector still rely on outdated methods like paper handouts, USB sticks, and DVDs to distribute resources. Think Self-Management recognized the need for a modern, scalable solution to provide healthcare professionals with easy access to evidence-based self-management tools for chronic conditions.

Approach
Discovery & Definition: We started by listening. I spoke with occupational therapists across Europe and paired those insights with a heuristic review and stakeholder interviews. Hearing clinicians describe their frustrations helped surface pain points and inspired practical recommendations for moving forward.
Ideation & Design: In remote workshops, we worked together to define product vision and requirements. A visual thinker, I mapped out processes, created wireflows, and built mockups and prototypes to spark conversation and test ideas. We took early versions to clinician conferences, gathering feedback that directly shaped the solution.
Implementation
With a clear design direction in place, I shifted gears into product management—developing the roadmap, running sprint planning, and guiding the first release. Once we launched, I focused on the bigger picture: a product strategy aimed at revenue growth, customer acquisition, and expansion across European markets, supported by a 12-month roadmap to keep momentum going.
The Outcome
The platform launched successfully, securing new funding and accelerating customer acquisition. It streamlined administration, improved the clinician experience, and positioned Think Self-Management for growth across European markets.
